The hospitals have to fill an application form for NABH Accreditation and the forms are available at NABH Secretariat or can be downloaded it from the web-site. The filed application form should be sent with the needed application fee. Hospitals should also attach a duly signed \u2018Terms and Conditions\u2019 document for obtaining NABH Accreditation\u2019.<\/p>\n
